When coaching stops, strength fades before muscle does

Everything works while someone is watching. The question is what happens next — and this trial built the "next" into the design: 12 weeks under supervision, then 12 weeks left alone at home, with the same measurements running throughout.
Participants were 62 adults aged 65 and over with both type 2 diabetes and sarcopenia (median 72 years, 67.7% women), randomized 1:1 to the 24-week program or to control. Four outcomes were tracked: skeletal muscle index, grip strength, five-times sit-to-stand time, and the SARC-CalF sarcopenia screening score.
What improved under supervision?
At 12 weeks, grip strength, five-times sit-to-stand, and the screening score had all improved significantly. The skeletal muscle index had not — no difference against control. That is the familiar order: twelve weeks is short for visible muscle gain and long enough to get better at using the muscle already there.
And after the supervision ended?
By 24 weeks, all three of those gains had attenuated. Over the same stretch, the muscle index moved modestly in the favorable direction. So the participants had not stopped training — they kept going, but without the intensity or precision that had been driving strength and function. HbA1c showed no clear change across the full 24 weeks.
Why does strength go first?
Grip strength and sit-to-stand respond to a heavy enough, precise enough stimulus. When supervision disappears, load selection and set intensity are the first things to blur. Muscle mass is a total with a lot of inertia; output reflects the last few weeks almost directly. Training alone costs you intensity before it costs you muscle.
So how do you hold the gains without a coach?
What the authors named in their conclusion was not more coaching hours but periodic check-ins, remote monitoring, automated reminders, and feedback-guided exercise logs. A log does part of the coach's job — when last week's weight and reps are in front of you, quietly dropping the load gets harder. That is the same reason a single converging score is useful. Whether adherence is an information gap or a support gap is covered in an information problem or a support problem, and the effect of visible comparison in competition beats encouragement.
